The Student Needs Assessment
The Student Needs Assessment (SNA) is a survey administered to students at our partnered schools that is designed to identify students who may not display outward signs of risk and experience barriers to accessing help. This assessment is a confidential process that is completed in school under the support of Upstream Youth and Family Workers. To discover more about the SNA, please click the button below.
Referral from School or Community Organization
Faculty of our partnered schools may identify students and families who may be able to benefit from the supports of the program. Members of Community Organizations who work closely with students attending our partnered schools may also submit a referral.
